You will need the Windows Icon plug-in for Adobe Photoshop.
1. Create an image with a 72 Pixel x72 Pixel size.
2. Use the pen tool to draw an image like this.

3. Go to "Paths" and right click your work path. Select "Make Selection." Don't change any settings. Just press "OK."
4. Make a New Layer. Fill the selection with Black.
It should look something like this.

5. Select the Work Path again. Edit it to look like this.

6. Repeat the same thing you did with the back side of the folder.
Blending Options for Backside and Front side-
Inner glow:
Blend Mode: Screen
Opacity: 40%
Noise: 0%
Color:#ffffff
Technique: Softer
Source: Edge
Choke: 100%
Size: 1 Pixel
Gradient Overlay
Blend Mode: Normal
Opacity: 100%
Color: #ffd46f to #fff29e
Reverse: No
Style: Linear
Angle: 90 Degrees
Scale: 100%
Stroke
Size: 1 Pixel
Position: Outside
Blend Mode: Normal
Opacity: 100%
Fill Type: Color
Color: #f4c65b
It should look something like this now:

7. Select the Front flap layer and go to Edit > Transform > Perspective.
8. Make the outer top and bottom higher. Making it look somewhat like a trapezoid.

9. Now go to Edit > Transform > Scale. Make the front flap come inside.
It should look something like this.
